Social Determinants of Health

Social determinants of health, or social drivers of health, are the personal or community-wide circumstances that influence health and well-being. Research shows that access to affordable housing, transportation, healthy food, quality education, and economic mobility are more instrumental in affecting health outcomes than healthcare alone.

CHF’s national network addresses social determinants of health through innovative approaches such as mobile health and transportation support, food pantries, school-based healthcare, legal aid services, and more.
