About the Project

The New Jersey Children’s Health Project (NJCHP) provides critically needed medical and case management services to children and families in two of New Jersey’s most underserved communities, Newark and Elizabeth.

The child poverty rate in both Newark and Elizabeth is very high; nearly one third of the children in these cities live in a household with income below the federal poverty level. This, compounded by high teen pregnancy rates and low levels of prenatal care, creates a dire situation for the children of these communities.

The New Jersey Children’s Health Project combats these challenges by providing comprehensive and continuous care, employing the medical home model as well as referrals to specialty and inpatient care, case management services, disease prevention, and education.
